カンファレンスの様子

そんぽの家小平仲町です。
本日10時からカスタムメイドケアカンファレンスがおこなわれています。
そんぽの家小平仲町では、第1・3木曜日にカンファレンス、第2・4木曜日にカスタムメイドケアカンファレンスを実施しています。
来月末でケアプラン更新の利用者様を対象に、普段のご様子や改善提案を話し合っています。
全ての入居者さまのケアプランに沿ったケアが行えるよう、またケアプランがPDCAサイクル(Plan(計画)→Do(行動)→Check(評価)→Action(改善))によって、その方の「できること」に着目する会議、QOL向上に向けての話し合いの場です。
普段のご様子は、実際に援助をおこなう私たちケアスタッフの日々の記録が重要になっています。
このカンファレンスに出席できなかったスタッフは、後日カンファレンスの議事録を確認して共有しています。
