いつもホームたよりをご覧いただきありがとうございます。
カスタムカンファレンス前に写真を撮りました!
皆様ノリが良く、いい笑顔ですね♪
これから会議なのに真剣味が足りないとは思わないでください。始まったらもちろん真剣です!
ちなみにカスタムカンファレンスは通常のカンファレンスと違い、ホーム長・副ホーム長・ケアマネジャー・看護師・介護リーダーなど
限られた人が参加します。
内容としては対象のご利用者様を毎回5,6人に絞り、介護職員から上がった事前情報をもとに、プランに沿って援助できているか、時間は適切か、ご本人様の様子はどうか?など
幅広い視点から話し合いを行います。多職種からの意見が聴ける為、より専門的な意見が飛び交います。
介護の現場では介護スタッフだけではなく、外部の方も含めて様々な方が関わり、ご利用者様を支えています。
私も微力ではありますが、これからも力になれるよう日々頑張っていきます!!
